Expanded In-Home Services for the Elderly
Program Details
The Expanded In-home Services for the Elderly Program (EISEP) provides frail older adults with access to a well-planned, coordinated package of non-medical in-home and other supportive services designed to complement and supplement informal care (i.e., care provided by friends and family). This program is for Schuyler County adults aged 60 and older who struggle to meet their needs independently. It is designed to serve a population of older adults who are not eligible for Medicaid but who do not have sufficient resources to pay for private care. Services are available on a cost-share basis according to income. A financial screen is completed. Some individuals may be eligible to receive the service free of charge.
Services
EISEP is designed to provide a variety of personal hands-on and household tasks coordinated to meet the needs of the individual client. These tasks may include dressing, bathing, cooking meals, laundry, grocery shopping, housekeeping chores, etc. An Office for the Aging case manager completes an assessment to determine an individual's eligibility, needed services, and the cost share amount, if any.
Ancillary Services
An individual may have unique needs that should be addressed in order to help the person remain safely in their home. Examples include assistive technology, adaptive equipment, personal supplies (e.g., incontinence products) or a Personal Emergency Response System (PERS), which allows access to emergency help with the touch of a button.
Case Management
EISEP includes case management services which are designed to help older adults assess their needs and develop, implement, and maintain an appropriate plan for services and how they are to be delivered. Case management brings order to the confusing array of services and benefits that an older adult might need. Case Management provides ongoing monitoring to help ensure that the plan is adequate, and that the individual is meeting his/her goals and is residing in the community.
All services are subject to budget limitations and availability.
